RELEASE OF LIABILITY WAIVER
 

Southern Starz Gymnastics is committed to conducting its activities in the safest manner possible. The safety and well being of all participants are of the highest importance. However, parents and guardians should recognize that participation in gymnastics, tumbling, cheerleading, camps, open gym activities, and related programs involves inherent risks that cannot be completely eliminated.
 

Assumption of Risk
 

I acknowledge that participation in activities offered by Southern Starz Gymnastics involves certain risks, including but not limited to falls, collisions, equipment-related incidents, sprains, strains, fractures, and other injuries that may occur despite reasonable supervision and safety precautions. I voluntarily accept and assume all risks associated with my child's participation in these activities.
 

Release and Waiver of Liability
 

In consideration of my child being permitted to participate in programs and activities offered by Southern Starz Gymnastics, I hereby release, waive, discharge, and hold harmless Southern Starz Gymnastics, its owners, officers, employees, instructors, coaches, volunteers, and representatives from any and all claims, demands, actions, causes of action, liabilities, damages, costs, or expenses arising from or related to any injury, illness, loss, or damage sustained by my child while participating in any Southern Starz Gymnastics activity, except as otherwise provided by applicable law.
 

Medical Authorization
 

In the event of an accident, injury, illness, or medical emergency involving my child, I authorize Southern Starz Gymnastics staff to obtain emergency medical treatment when immediate action is deemed necessary and a parent or guardian cannot be reached. I understand that every reasonable effort will be made to contact a parent or guardian as soon as possible. I further acknowledge that I am financially responsible for any medical expenses incurred as a result of such treatment.
 

Participant Responsibilities
 

I understand that participants are expected to follow all gym rules, safety procedures, and instructions provided by Southern Starz Gymnastics staff. Failure to comply with established rules or safety requirements may result in suspension or removal from participation at the discretion of Southern Starz Gymnastics.
 

Indemnification
 

I agree to indemnify and hold harmless Southern Starz Gymnastics, its owners, employees, instructors, coaches, volunteers, and representatives from any claims, damages, losses, liabilities, or expenses, including reasonable attorney fees, arising from my child's participation in any Southern Starz Gymnastics activity.
